Diagnosis of perianal cellulitis.

Clinical Information

Streptococcus pyogenes (group A Streptococcus) is a common cause of pharyngitis and skin and soft tissue infection. In children, S pyogenes can cause perianal infection. The classic presentation is a well-demarcated rash around the anus with itching, rectal pain and occasionally, blood -streaked stools. Untreated, painful defecation, toilet avoidance and constipation may persist for months, until effective treatment is administered. Anal fissures may ensure. A swab of the affected area may be submitted for S pyogenes culture to confirm diagnosis.

Specimen Requirements

Specimen must arrive within 48 hours of collection.

 

Whenever possible, culture specimens should be obtained before antibiotics or antimicrobial agents have been administered.

 

No direct smear performed.

Collection Instructions:

1. Dry swab is not acceptable.

2. Collect anorectal specimen using a sterile culture swab. Specimen should consist of mucopurulent material obtained from crypts just inside anal sphincter. Avoid contamination with fecal material.

1. Label tube with patient’s name (first and last), date and actual time of collection, and source/type of specimen.

2. Specimen source is required.
