Additional Codes

Test Name in EPIC EPIC Test Code Mnemonic
STREP PNEUMO ANTIGEN LAB6061 SPNAG

Useful For/Utility

The Immuview Urinary Antigen Test is a qualitative assay intended to aid in diagnosis of Streptococcus pneumoniae serogroup 1 infections in the urine of patients with symptoms of pneumonia.

The Immuview CSF Antigen test is a qualitative assay effective in presumptive diagnosis of Streptococcus pneumoniae and should be interpreted in conjunction with the patient’s clinical evaluation and other diagnostic methods.

Methodology

Immunochromatographic Assay

Clinical Information

Streptococcus pneumonia is the most frequently encountered bacterial agent of community-acquired pneumonia (CAP), and can also be an agent of bacterial meningitis. Because of the significant morbidity and mortality associated with pneumococcal pneumonia, septicemia, and meningitis, it is important to have diagnostic test methods available that can provide a rapid diagnosis. In instances where empirical antibiotics are provided for CAP without culture, or being considered prior to culture confirmation of S pnuemonia in spinal fluid, antigen testing may be useful.

Specimen Requirements

This test should not be performed within 5 days of receiving Streptococcal pneumoniae vaccination.

Specimen source is required.

Submit only 1 of the following specimens:

Specimen Type: Spinal fluid

Container/Tube: Sterile vial

Specimen Volume: ≥1 mL

Additional Information: Label vial with patient’s name (first and last), date and actual time of collection, and type of specimen. CSF submitted for testing must be accompanied by a culture, LAB7016-Culture, CSF.

Specimen Type: Urine

Container/Tube: Sterile container

Specimen Volume: 1 mL

1. Collect urine into a screw-capped, sterile container. Mix well.

Additional Information: Label container with patient’s name (first and last), date and actual time of collection, and type of specimen.

East Region: SMDC Clinical Lab

Test Classification and CPT Coding

87449

Reference Values

Negative (reported as positive or negative)

Additional Information

Pneumococcal pneumonia is best diagnosed by sputum culture.

Interpretive Data

Positive result is indicative of pneumococcal pneumonia, or pneumococcal meningitis. Negative result is negative for pneumococcal pneumonia or that the pneumococcal antigen is absent in spinal fluid, suggesting no current or recent pneumococcal infection. Infection due to Streptococcus pneumonia cannot be ruled out since the antigen present in the sample may be below limit of test.
