Useful For/Utility

Diagnosing congenital deficiency (rare), evaluating acquired deficiencies associated with liver disease or vitamin K deficiency, oral anticoagulant therapy, and antibody-induced deficiencies (eg, in association with lupus-like anticoagulant), determining warfarin treatment stabilization in patients with nonspecific inhibitors (ie, lupus anticoagulant), determining degree of anticoagulation with warfarin to correlate with level of protein S, or investigation of prolonged prothrombin time or activated partial thromboplastin time.

Clinical Information

Factor II (prothrombin) is a vitamin K-dependent serine protease synthesized in liver. It participates in the final common pathway of coagulation, as the substrate for the prothrombinase enzyme complex. Prothrombin is the precursor of thrombin (IIa), which converts fibrinogen to fibrin. Plasma biological half-life is about 3 days. Deficiency of factor II may cause prolonged prothrombin time and activated partial thromboplastin time. Deficiency may result in a bleeding diathesis.

Specimen Requirements

Specimen Type: Platelet-poor plasma

Container/Tube:

Fill Blue top tube to the etched line located just below the cap of the tube.

Preferred: Full Light-blue top (3.2% sodium citrate)

Acceptable: Full Sodium Citrate (BD® or Greiner tubes

Specimen Volume: 1 mL

Specimen Minimum Volume: 0.5 mL

Collection Instructions:

1. Do not spin in refrigerated centrifuge.

2. Unacceptable specimens: Specimens that are short-draws, over-draws, clotted or hemolyzed may yield incorrect results.

3. If sending from a site spin down, remove top 2/3 of plasma, and spin plasma again in a polypropylene (cloudy plastic) vial.

4. Aliquot top 3/4 and place platelet-poor plasma in a polypropylene (cloudy plastic) vial.

5. Glass vials are not acceptable for processing/transport/storage.

Additional Information:

1. Double-centrifuged specimens are critical for accurate results as platelet contamination may cause spurious results.
2. If there is a delay in transport of > 4 hours, send specimen frozen.

*Note: If the same plasma is also to be used for testing of Factor VII, do not store at 2-8° C.
