Useful For/Utility

Fresh Frozen Plasma/Thawed Plasma transfusions are indicated:
1. PT:INR > 1.7 with active bleeding
2. PTT > 1.5 X upper limit of normal with active bleeding
3. Replacement for therapeutic plasma exchange
4. Coagulation factor replacement in Massive Transfusion Protocol
5. Reversal of warfarin therapy with INR > 1.7 and major bleeding or impending surgery: if time permits, vitamin K should be used. If urgent reversal is needed, FFP or Four Factor PCC can be used. Reference Pharmacy Protocols.
6. C1-esterase inhibitor replacement (if specific factor concentrate is not available)
7. Treatment of specific factor deficiency (if specific factor concentrate is not available)

Test Algorithm

An ABO/Rh must be completed within six months of the planned transfusion.

Clinical Information

Essentia Health labels all FFP products as Thawed Plasma once thawed. Thawed Plasma is FFP, PF24, or PF24RT24 that has been thawed and held at refrigerated temperature for up to 5 days from the date of thaw. Thawed Plasma has acceptable factor levels with the exception of Factor V, and Factor VIII and is typically used to avoid delays associated with thawing in emergency situations.

Call Transfusion Services to request this test. Allow 25 minutes minimum for thawing.

1. Blood type and Rh are on file before preparation.

2. Product expires 5 days after thawing.
