Useful For/Utility
Platelet transfusions indicated:
1. PLT count < 10 X 10 ˄9 /L in stable, nonbleeding patients
2. PLT count < 50 X 10 ˄9 /L with bleeding or pre-procedure, or for neonates
3. PLT count < 100 X 10 ˄9 /L with bleeding requiring RBC transfusion, or neuraxis procedures or ocular procedures
4. Bleeding, regardless of platelet count, due to a platelet dysfunction when there is an abnormal platelet function test or documentation of a clinical history indicative of platelet dysfunction (eg. Inherited qualitative disorder; following ingestion of aspirin or platelet inhibitors such as Plavix [clopidogrel bisulfate]; or chronic uremia)
5. Platelet replacement in Massive Transfusion Protocol
6. Prophylactic platelets are not recommended for: TTP, Post Transfusion Purpura, Hemolytic Uremic Syndrome, HELLP Syndrome, Ticagrelor use. Exception: clinically significant bleeding.
Test Algorithm
An ABO/Rh must be completed within six months of the planned transfusion. Patients who have a documented lack of response to random donor apheresis platelets may benefit from more closesly selected products. ABO identical, crossmatched, or HLA matched platelets may be indicated when at least two post transfusion platelet counts have been performed and indicate and inadequate platelet response. Essentia Health keeps Apheresis platelets routinely on hand at our three larger locations, St. Marys Medical Center in Dultuh, Fargo 32nd Avenue Hospital, and St. Josephs Medical Center in Brainerd. All other locations can obtain them upon request. Platelets have a 5-7 day outdate depending on the blood supplier and have a much shorter life span by the time they are received in the transfusion service. Orders to "Hold" platelets will not be accepted, as we have locations that keep minimum quantities and all other locations order on a "to transfuse" basis.
Clinical Information
Platelet Apheresis contain at least 3 X 10 ˄11 platelets. Units containing less than that are considered low yield and will be labeled with the actual platelet count. Low yield platelets can be used at the discretion of the physician, but they are not routinely stocked at this time.
Specimen Requirements
Call Transfusion Services to request this test. Include special requirements (ie, Irradiation).
Additional Information:
1. Patients blood type and Rh are required on file with Transfusion Services.
2. Pheresis products will be provided, if available, or ordered from the blood supplier.
3. Pheresis product is equivalent to 6 to 8 pooled platelet packs.
4. Pooled platelets expire 4 hours after pooling.
