Useful For/Utility

The FMH test is indended only for determining RhIG dose. It is NOT intended for the use of detecting a fetal bleed in a maternal patient when the fetal type is unknown, as in abdominal trauma. In those cases a Hemoglobin F or Kleihauer-Betke stain should be used.

Methodology

Rosette Test

Screening test for feto-maternal hemorrhage in Rh-negative women with a known Rh-positive infant. Prior to delivery, order a Kleihauer Stain (LABKLIEH).

Test Algorithm

A positive FMH will require quantitative testing (Hgb F or Kleihauer-Betke) to determine the appropraite RhIG dose. An FMH is performed on all RhIG candidates and must be collected prior to the first dose of RhIG given.

The Fetal Maternal Hemorrhage (FMH) is a qualitative test to detect fetal maternal hemorrhage in Rh negative females with Rh positive infants.
