Useful For/Utility
An Anal Pap test is not a diagnostic test, but rather looks for precancerous and cancerous conditions in the anus.

Clinical Information
An Anal Pap test is a screening test for anal cancer in patients of any age. Also called anal cytology, it's performed by taking a sample of cells from the anus that are then examined under a microscope in a lab. If present, organisms can also be seen such as fungal, microorganims, etc. HPV testing can be performed on the same sample.
Specimen Requirements
For best results, specimen must arrive as soon as possible after collection.
Specimen source is required.
Specimen Type: Monolayer anal pap test collection
Container/Tube: SurePath vial
Specimen Volume: Dacron/Polyester swab in liquid media (SurePath vial) *Alternate Method of Collection: Rovers Cytobrush
Collection Instructions:
1. Obtain an adequate sampling of anal transformation zone by inserting a water moistened Dacron/Polyester swab into the anal canal 5-6 cm.
2. Using gentle pressure, rotate the swab to sample all sides of the anal canal.
3. After collecting the specimen, place the swab directly into the SurePath vial, vigorously swirling the swab while pressing against the wall of the vial. Break or cut the tip of the collection device off and leave it in the SurePath vial. For a more cellular specimen, you may repeat this collection a second time with a new Dacron/Polyester swab and place it into the same SurePath vial.
4. Submission of the collection device(s) is preferred in the SurePath vial.
5. Cap must be straight and tight.

Testing Algorithm
Note: This test can be ordered on any age patient.
In order to ensure reflex HPV testing, you must answer the HPV Reflex? question:
- NEVER: No HPV test will be performed
- ALWAYS: HPV testing will be performed
- If ASCUS: HPV testing will be performed if the interpretation is Atypical Squamous Cells of Undetermined Significance or Atypical Glandular Cells, Not Otherwise Specified.

An interpretive report will be provided.
Additional Information
Container must have the patient's full name, Medical Record Number or Date of Birth, provider's name, date and time of collection, and specimen source. An EPIC order label is preferred.
The following specimens are at risk of being delayed and/or discarded:
-Spilled specimen
-No requisition form or EPIC order
-Name on requisition or EPIC order does not match name on specimen
-Unlabeled specimen
-No provider name given
Place specimen in a plastic specimen bag with request form (if present) inserted into the side pocket separate from specimen.
