Useful For/Utility
Fluorescent dye tagged monoclonal antibodies are used to identify abnormal hematologic cell populations including leukemia and lymphoma. Flow cytometry testing is recommended in the following scenarios:
1. unexplained pancytopenia, morphologic detection of atypical cells or blast.
2. leukocytosis consisting primarily of lymphocytes, monocytes, or eosinophils
3. plasmacytosis or monoclonal gammopathy
4. lymphadenopathy, tissue masses or organomegaly
5. monitoring of patients with known hematopoietic malignancy (staging, evaluation for expression
of therapeutic targets, minimal residual disease detection.
Methodology
Immunophenotyping by Flow Cytometry
Test Algorithm
Specimens are screened to identify abnormal cell populations. Additional panels are added if an abnormality is detected, if the morphology suggests acute leukemia, or if the patient has a history of a hematologic malignancy.
Clinical Information
Leukemia and lymphoma immunophenotyping is a valuable and important complementary tool to morphologic assessment of blood smears, bone marrow smears, and tissue sections. Immunophenotyping hematopoietic specimens can help resolve many differential diagnostic problems posed by the clinical or morphologic features of hemotologic disease states. This test is appropriate for hematopoietic specimens only.
Specimen Requirements
Specimen source, date of collection, and an order comment stating the reason for request including pertinent patient history is required at order entry.
Specimen Type: Bone marrow aspirate + 2 unstained bone marrow aspirate smears
Container/Tube: Yellow top (ACD solution B) is preferred. Lavender top EDTA or green top Sodium heparin are acceptable.
Specimen Volume: 1 mL
Collection Instructions:
1. Mix well. Clotted bone marrow aspirate specimens are undesirable.
2. Send specimen in original tube.
3. Forward promptly to Flow Cytometry Lab at ambient temperature. Specimens should be received in the Flow Cytometry Lab within 24 hours of collection or as soon as possible thereafter. It is recommended that testing be completed within 48 hours of specimen collection for optimal cellular viability.
4. Add-on flow cytometry testing to specimens that have been refrigerated is acceptable, however alterations in antigen expression may be seen following refrigeration. Specimens cannot be frozen.
