Pain Management Drug Screen with Interpretation by High-Resolution Time-of-Flight Mass

To submit medication information, use one of the following methods:
1. Legible paper copy included with packing slip
2. Medication information placed in Order Notes
3. Medication information placed in Order Comments
4. Medication information placed in Drugs Expected test field
Information that can be included:
1. Trade name
2. Generic name
3. Dosing frequency (PRN, bid, etc.)
4. Date of last known dose taken
Please note that if no medications are provided, the following interpretation will be supplied:

Please call ARUP Laboratories Client Services at (800) 522-2787 for medical director interpretation if
applicable. Alternatively, please consider the PAIN HYB U (ARUP test code 2007479), which does not
require medication information or provide compliance interpretation.
If no drugs are expected, please use one of the following descriptions to allow for accurate
interpretation:
1. No drugs expected
2. No prescribed medications
Ideally, drug classes (e.g., benzodiazepines, opiates, or illicit) should not be submitted; specific
medication information is required.
