Useful For/Utility
Titration studies are used to estimate antibody activity in alloimmunized pregnant women to determine whether and when to perform more complex invasive investigation of the fetal condition, such as amniotic fluid analysis, RUBS, or cerebral artery velocity. These procedures are used to determine the need for intrauterine transfusion or early delivery.

Test Algorithm
Antibody titer will include antibody identification and appropriate red cell antigen typing(s) as required. All clinically significant abitodies identified in a prenatal patient will reflex to an antibody titer. The Kell antibody can supress production of RBCs in the fetus and severity of the caused anemia is not related to the titer. Special considerations should be taken with managment of Kell sensitized patients. M antibodies can be either IgG or IgM, but rarely cause HDFN. Anti-P1, -I, -Lea, and -Leb have not been reported to cause HDFN and no titer is needed. If the antibody is no longer reacting in gel, performance of an antibody titer is not indicated.

Titration is a semiquantitative method used to determine the concentration of antibody in a serum sample or to compare the strength of antigen expression on different red cell samples.
