Specimen Type: Nerve tissue

Container/Tube: Sterile container

Specimen Volume: 4.5 cm in length for teased fiber analysis, epoxy embedding, paraffin embedding, and frozen.

Additional Information:

1. Label container with patient’s name (first and last), medical record number, date and actual time of collection, physician’s name, and type of specimen.

2. Deliver specimen immediately to SMDC Clinical Laboratory Specimen Processing.

Forms: Anatomic Pathology Consult Requisition and Surgical Pathology Requisition including suspected clinical diagnosis and a brief clinical history.
