Clinical Information
Daratumumab is an IgG monoclonal antibody that recognizes CD38 on myeloma cells and on red blood cells. Sarclisa binds CD38 on RBCs. Both drugs present as a passively acquired IgG antibody that will react with all reagent red cells at the IAT phase of testing. Interference from these drugs can last up to six months after therapy has stopped.

Test Algorithm
Failure to request this orderset prior to the first dose of either drug will likely cause transfusion delays, if transfusion is indicated in course of treatment. Due to the nature of interference with our reagent red cells, the work up may need to be sent out to a larger reference lab for DTT/AET screening cells.
